When submitting this form, you will need to provide a GP Patient Summary and an Occupational Therapy Housing Needs Assessment from Nottinghamshire County Council.

Please complete this form if you need to move out of your current accommodation due to mental health needs.
When submitting this form, you will need to provide evidence from a medical professional, such as a GP or consultant doctor, which confirms:
- Any diagnosed mental health conditions you have, and
- What impact your current accommodation is having on your mental health, and
- What impact moving into alternate accommodation will have on your mental health.
